Privacy Policy
Who this covers
This policy explains how JT Halo Healthcare ("JT Halo," "we," "us") handles information collected through this website. Protected health information created or received in the course of treatment is governed separately by our Notice of Privacy Practices under HIPAA, summarized below and available in full on request.
Information we collect
You give us: name, email address, phone number, preferred contact method, appointment interest, and anything you choose to write in the contact form.
Collected automatically: standard server logs such as IP address, browser and device type, pages visited and referring site, kept by our hosting provider for security and performance. We do not use analytics or advertising trackers on this site.
We do not knowingly collect health information through this website, and we ask you not to submit it. If you do, we treat it with the same confidentiality as clinical information, but the form itself is not a secure medical record.
How we use it
To respond to your inquiry, schedule and confirm appointments, verify insurance eligibility, meet legal and professional obligations, and maintain the site. We do not sell your information. We do not use it for advertising.
Who we share it with
- Service providers who operate the site, host data or transmit email on our behalf, bound by contract to protect it, including our website hosting provider and email provider.
- When required by law: court order, subpoena, licensing board inquiry or mandatory reporting obligation.
- To prevent serious harm: where disclosure is permitted or required to protect you or another person from imminent danger.
Summary of our HIPAA Notice of Privacy Practices
As a healthcare provider, we may use and disclose your protected health information for treatment, payment and healthcare operations without separate authorization. Most other uses require your written authorization, which you may revoke at any time.
You have the right to inspect and receive a copy of your record; request corrections; receive an accounting of certain disclosures; request confidential communications by alternative means; request restrictions on use and disclosure; receive notice of a breach; and receive a paper copy of the full Notice.
To exercise any of these rights, or to file a complaint, contact us at info@jthalohealthcare.com or (781) 604-6914. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing one.
Data retention
Website inquiries are retained only as long as needed to respond and follow up. Clinical records are retained for the period required by Massachusetts law and professional standards.
Your choices
You may opt out of non-essential email at any time using the unsubscribe link or by contacting us. Most browsers let you refuse cookies; the site functions without them. We honor Global Privacy Control signals where technically supported.
State privacy rights
If you are a resident of a state with a comprehensive privacy law (including California, Colorado, Connecticut and Virginia, among others), you may have the right to access, correct, delete or obtain a portable copy of personal information we hold, and to appeal a denial. Contact info@jthalohealthcare.com to exercise these rights. Information covered by HIPAA is generally exempt from these laws and is handled under the HIPAA process above.
Children
This site is intended for adults. We do not knowingly collect information from anyone under 18.
Security
We use industry-standard safeguards including encryption in transit. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.
Changes
We may update this policy. Material changes will be posted here with a revised effective date.
